Forms for Visiting Artists: Chapter Report

There are two forms for Visiting Artists to complete after MAP events.  Please fill out the applicable forms submit them by May 1:

  • As applicable, please speak to noise, acoustics, temperature, lighting, writing table, general comfort, etc.
  • Adjustable height, footstool for small children, tuning etc.
  • As applicable, please speak to turning out the lights, unlocking/locking the facility, turing on and off heat, etc.

  • Scheduling

  • Please provide comments as applicable. For example, was the facility free of outside interference and distractions?
