Before completing this form:

The chapter chair (or other chapter organizer) should communicate with the student(s) in the chapter to determine who will be available to perform at the Honors Recitals in June, as only these students are to be reported on this form.

You will need the following for each Honors Recitalist:

the email address and phone number of the parent/guardian
the name and composer of the piece of music
the teacher’s name and email address
if required, the collaborative artist’s name and email address

Your chapter is allowed to have THREE students perform in June Honors Recitals at the WSMTA Conference at Lower Columbia College in Longview.

Your chapter is allowed to have TWO students to perform in June Honors Recitals at the WSMTA Conference at Lower Columbia College in Longview.

Your chapter is allowed to have ONE students perform in June Honors Recitals at the WSMTA Conference at Lower Columbia College in Longview.

If you do not agree with the number of students allowed to perform from your chapter, please contact the WSMTA Administrative Coordinator.

The chapter chair has confirmed that this student has agreed to participate at one of the honors recitals at the 2025 WSMTA Conference at Lower Columbia College in Longview WA from June 28-30, 2025.
If this student learns before April 22, 2025 that they are unable to participate in one of the honors recitals at the 2025 WSMTA Conference, they will contact the chapter chair and the state Honors Recital Chair. The chapter chair will communicate with an alternate performer from their chapter and submit the name of the alternate performer from their chapter by April 22, 2025.
Any confirmed student recitalist who cancels after April 22, 2025 will be considered a no-show.
